GENERAL VERIFICATION PROCESS
Step 1 - SUBMIT
Collate all the documents into one PDF file and email them to
polomilan1@gmail.com for initial evaluation
Step 2 - INTERVIEW
The Labor Attaché will conduct an interview with the
Employer or Authorized Representative
Step 3 - PAY
Submit the original documents and 2 additional
copies and pay for the verification fee
Step 4 - PICK-UP
Pick-up the documents and retain one (1) copy with POLO.
You may now submit the verified documents with POEA through your agency.
REQUIREMENTS

- Notarized Employment Contract (one original and one photocopy)
- Undertaking (one original and one photocopy)
- 2 copies of worker’s passport
- 2 copies of worker’s work permit
- 2 copies of valid ID or passport of the direct employer
- Cover Letter (Addressed to POEA administrator thru the Labor Attache. Must include reason for exemption from the No-Direct hiring policy)
- Employment Contract (preferably POEA Standard Employment Contract)
- Copy of Worker’s Passport
- Copy of Approved Nulla Osta or Visa if available
- Copy of Employer’s Business Registration (if not English, include a copy of the notarized translation)
- Passport Copy of Employer or Valid ID of Company Representative (contract signatory)
- Copy of Worker’s Diploma
- Copy of Workers Resume with detailed training and seminars attended and work experience
- Copy of Job Offer/Job Description from Employer
- Notarized Manning Agreement
- Notarized Special Power of Attorney
- Crew Request (Indicating positions, number of persons required, salary and other emoluments per position.)
To be prepared per vessel.
- Notarized Master Employment Contract for Seafarers
- Valid POEA License of Philippine Manning Agent/Company
- Valid identification of Philippine Manning Agent Rep/Signatory
- Business Registration – Foreign Principal/Employer/Vessel Owner
If not in English, a notarized English Translation must be provided.
- Valid identification of Employer/Authorized Signatory
If Authorized Signatory, submit Authority by Company/Board
- Vessel Registration and Particulars
- Summary list of Vessels for enrollment (IMO Number, registry,classification…)
- Undertaking (Manning Agent and Employer/Principal)
- Copy of Certificate of Cover/Compulsory Insurance
- Management Agreement (If Principal is not the Vessel Owner)
- Notarized Recruitment Agreement/Service Agreement between Principal and Philippine Recruitment Agency (PRA)
- Notarized Special Power of Attorney
- Job Order (Directed to PRA indicating number of positions required and salary per position)
- Notarized Master Employment Contract signed on all pages by the Principal or his authorized representative
- Valid POEA License of Philippine Recruitment Agency (PRA)
- Valid identification of PRA representative
- Business License or Commercial Registration (If not in English, a notarized English Translation must be provided
- Valid identification of Employer/Authorized Signatory (If Authorized Signatory, submit Authority by Company/Board)
- Employer Profile (Business Activities, Number of years in operation, Labor force volume)
- Undertaking
- Contingency Plan
- Recruitment Agreement / Service Agreement Between FPA and Philippine Recruitment Agency (PRA)
- Special Power of Attorney
- Job Order ( FPA to PRA indicating number of positions required and salary per position )
- Master Employment Contract signed on all pages by the FPA or his/her representative
- Joint Undertaking – signed by both PRA and FPA
- Contingency Plan
- Valid POEA License of Philippine Recruitment Agency (PRA)
- Valid identification of PRA representative
- Business License or Commercial Registration of FPA (If not in English, a notarized English Translation must be provided)
- Valid identification of authorized signatory of FPA
- License to operate as an Employment Agency from host country
- Manpower Request (By Direct Employer/s to FRA indicating number of positions required and salary per position)
- Master Employment Contract jointly signed on all pages by the FPA and actual employer
- Business Registration of actual Employer
- Service Agreement between the FPA and actual employer
- Undertaking by the FPA/employer to monitor the employment of Overseas Filipino Workers and to submit a report of significant incidents relative thereto.
